Target Audience: Pharmacists who practice in managed care settings.
As the number of people in the United States with diabetes continues to grow—and the number of available treatment options multiplies—diabetes presents increasing challenges for managed care. Is it possible to ensure optimal disease management, prevent complications, and maximize quality outcomes while simultaneously controlling costs? At Capital District Physicians’ Health Plan, Inc. (CDPHP) in Albany, New York, the answer is yes—by using a coordinated approach that combines strategic formulary decisions with consistent clinical messaging to internal and external stakeholders.
In this session, a pharmacist and physician engagement specialist from CDPHP share their experiences creating and deploying formulary changes and other interventions to meet the challenges of diabetes management.
At the completion of this activity, participants should be able to:
- Recall current evidence-based recommendations for the treatment of diabetes.
- Describe a cost-conscious approach to evaluating new and emerging diabetes medications (including biosimilar insulin) that incorporates clinical comparison data, evidence-based guidelines, and quality metrics.
- Identify trends and opportunities for controlling costs associated with diabetes medications.
- Discuss ways of engaging the physician community to maximize awareness and implementation of pharmacy initiatives related to diabetes management.

This activity consists of an audio recording, pre-test assessment questions (not required to pass or continue), post-test questions (required to pass with a score of 70% or higher) and a specific activity evaluation.
