Activity Overview

Why all the buzz about the Enhanced Medication Therapy Management (eMTM) Model? What makes it different from the traditional Part D MTM model? How does it address the value of medications in managing chronic disease? Starting in 2017, the Enhanced MTM model program will provide Medicare Part D sponsors with new flexibility and incentives to design innovative MTM programs that optimize medication use, improve care coordination, and strengthen system linkages. Will it improve patients’ quality of life and reduce total cost of care? 

Attend this session to learn about the model objectives, gain insights into early innovations developing in the market, and get an update on the use of SNOMED CT for standardized data collection and reporting—the standard for evaluating Enhanced MTM and the potential foundation for pharmacy’s future interoperability within the health care system. The best practices identified during this model test have far-reaching implications both within and beyond Medicare Part D MTM programs. 

Learning Objectives:  

At the completion of this activity, participants should be able to:

1. Summarize the evidence supporting the Enhanced Medication Therapy Management (eMTM) Model for improvements in patient care and reduced payer costs.

2. Explain the eMTM model objectives and key elements for its implementation and use.

3. Describe the MTM Encounter Data requirements and the implementation of SNOMED CT for integration of the eMTM model.

4. Discuss insights into early innovations of the eMTM model.

This activity consists of an audio recording, pre-test assessment questions (not required to pass or continue), post-test questions (required to pass with a score of 70% or higher) and a specific activity evaluation.
