The intersection of population health and managed care pharmacy becomes clearer when viewed through real-world application. Building from foundational concepts introduced in the first webinar, this session explores how population health principles are implemented across diverse settings, including employer groups, health plans, research organizations, and healthcare systems. Through case studies and practical examples, speakers will demonstrate how professionals with different roles and titles contribute to shared goals of improving health outcomes, enhancing patient experiences, and optimizing healthcare resources. Attendees will gain insight into how population health strategies are translated into action and discover opportunities to apply similar approaches within their own organizations.
