Course Description
This educational initiative is targeted to managed care pharmacists and other managed care professionals. Designed as an adaptive, timely, and engaging session led by patient-facing diabetes care experts, this activity provides managed care professionals a comprehensive exploration of the current quality measures in diabetes management, with an emphasis on best practices for using various monitoring devices to meet these metrics. The critical role of the managed care professional in aligning managed care and provider clinical decision-making with current evidence and patient-specific factors to ensure appropriate management of diabetes is a central theme and a point of incisive expert focus throughout the activity. Practical insights from expert faculty on overcoming healthcare disparities and inequities that can adversely affect patient outcomes in diabetes are discussed, tangibly highlighting the imperative need for interprofessional team-based collaboration and the importance of individualized, data-driven, and equitable treatment selection. Real-world case-based scenarios are employed throughout the content to accentuate the practice-changing value of existing and emerging monitoring technologies in the optimization of diabetes care.
